Good Shepherd Hospice is seeking a compassionate, entry-level Registered Nurse Case Manager (RN) to support hospice patients and their families in Newton, KS and nearby areas. This hybrid role allows you to work remotely with local field visits to ensure patients receive the highest level of care during life’s most meaningful moments.

đź’Ľ Key Responsibilities

  • Deliver compassionate nursing care that maximizes patient comfort and dignity

  • Perform initial and ongoing assessments of patient conditions

  • Develop and implement individualized care plans

  • Educate caregivers on medication administration, personal care, mobility assistance, and signs of approaching death

  • Maintain regular contact with patients, families, and the attending physician

  • Respond to urgent needs, after-hours admissions, and patient deaths as needed

  • Participate in interdisciplinary team meetings and care planning sessions

  • Provide emotional and practical support at the time of death

  • Complete and submit documentation per Medicare and hospice policies

  • Evaluate hospice aides bi-weekly and LPNs monthly

  • Remain on-call as part of a shared coverage schedule (24/7 rotation)

âś… Required Qualifications

  • Active RN License in Kansas (or compact state equivalent)

  • Valid driver’s license and reliable transportation for local visits

  • Excellent communication, assessment, and time management skills

  • Ability to work independently while remaining connected with a remote team

  • Comfortable with electronic charting and documentation

  • Experience in hospice, home health, or geriatrics

  • Strong understanding of hospice philosophy and palliative care

  • Training or interest in pain management, bereavement care, and end-of-life planning
