Position: Field Assessment Nurse

Location: Bronx & Westchester Counties, NY
Schedule: Full-Time, Monday–Friday, 8:30 AM – 5:00 PM EST
Sign-On Bonus: $5,000 (for external candidates)
Hourly Pay Range: $28.03 – $54.95 (based on experience and qualifications)

What You’ll Do

As a Field Assessment Nurse, you’ll play a vital role in coordinating long-term care for patients within their communities. You’ll assess patients’ health needs, develop care plans, and act as an advocate to ensure members receive the care they deserve. Flexibility is offered for New York City residents to work remotely, while travel within the five boroughs and Westchester County is required.

What We’re Looking For

Required Qualifications:

  • Current, unrestricted RN license in New York State or ability to obtain prior to hire

  • 2+ years of clinical nursing experience

  • 1+ year community case management experience with complex patient needs

  • Knowledge of home health, hospice, public health, or assisted living settings

  • Proficiency in MS Word, Excel, Outlook

  • Ability and willingness to travel locally to patient homes and community locations

Preferred Qualifications:

  • Behavioral health or clinical degree

  • Experience with electronic charting and community resource coordination

  • Previous field-based nursing or population health experience

  • Familiarity with UASNY assessments
