🛠️ Your Responsibilities Will Include:

Conducting in-home or virtual assessments using the NYS UAS tool to develop individualized care plans
Coordinating long-term care needs through a person-centered approach
Managing referrals for financial, social, and community-based services
Acting as a single point of contact for patients throughout their care journey
Communicating effectively with care teams and advocating for the member’s needs

✅ What You’ll Need:

Required Qualifications:

Active, unrestricted RN license in the State of New York
2+ years of clinical nursing experience
1+ year of community case management experience
Knowledge of home health, hospice, assisted living, or public health systems
Proficiency in Microsoft Office (Word, Excel, Outlook)
Reliable transportation and willingness to travel within the 5 boroughs or Westchester County

Preferred Qualifications:

Familiarity with UASNY and electronic charting systems
Experience working with behavioral health or medically complex populations
Previous field-based nursing experience
Bilingual fluency is a plus (not required)
